Kratom is a tropical tree (Mitragyna speciosa) native to Southeast Asia. Its leaves that contain compounds that can have psychotropic (mind-altering) effects. Most people take kratom as a pill, capsule, or extract. Some people chew kratom leaves or brew the dried or powdered leaves as a tea. The leaves can also be smoked or eaten in food.
Kratom can cause effects similar to both opioids and stimulants. In large doses, the compounds mitragynine and 7-α-hydroxymitragynine in kratom interact with opioid receptors in the brain, producing sedation, pleasure, and decreased pain. In small doses, users report increased energy, sociability, and alertness instead of sedation.
Kratom is not currently an illegal substance and it is easily obtained. Because of its potential serious side effects and addictive nature, several states have banned kratom products. It is a controlled substance in Thailand, Malaysia, Australia, and some European countries. The U.S. DEA has proposed banning kratom federally.
Kratom has been on the DEA’s list of drugs/chemicals of concern due to the number of deaths it caused. In 2017, the FDA identified at least 44 deaths related to kratom. FDA reports note that death from kratom is a result of adulterated products or taking kratom with other potent substances.
Since Kratom is still not banned in USA, there is still widespread abuse of the drug. 2000 to 2005 saw two exposures to Kratom, but in 2010-2015, 660 calls to US poison centers was due to kratom exposure. Between 2014 and 2016, 12 million doses of the drug have been discovered at various ports of entry into the US.
